3390 – CSIR Blue Card

Family Services Staff will print a School Immunization Record on blue paper for each child on your caseload and ensure it is kept up to date throughout the entire program year.

Type 3390 in the Search Reports Box, select the report titled 3390 California Immunization Record

Program Term:  “Your Program” (EHS/HS) and current program year

Location: Shasta Head Start Child Development, “Your Site”

Enrollment Status: Enrolled

Flag/Group: Not Filtered

Program Option: All

Responsible Staff: All

Custom Filter: Not Filtered

Click View

To print one or more children’s blue cards click on the individuals tab. Select only the children you want to print and select View.  You will then be able to print the Immunization Record you need.

Always Preview your report before Printing.
