Mental Health (ASQ-SE)
Ed staff must complete the ASQ-SE 2 screener within 45 days of entry into each program option.
Enter within 3 days of completion.
Select The Participant from the Participants List in the Services Tab, select the Health Tab, and then select the Events section

Click on the Green “Add Event” button, then select the “Mental Health (ASQ-SE)” Health Event

Scheduled Date: Blank
Event Date: Enter date the screening was completed
Status: Enter appropriate status code:
Colors refer to the chart on summary page of the ASQ-SE
Passed First Screening: Scores within the White area on the ASQ-SE results page- Enter Closed Date using the same date as the Event Date
Needs Re-Screening, Failed 1st: Scores within the Gray area on the ASQ-SE results page-Follow the instructions for Mental Health (ASQ-SE) Follow Up to add a scheduled Follow Up action 30 days after initial screening date.
Failed 1st Screening, Needs Referral: Scores within the Black area on the ASQ-SE results page, Follow the Referral for Observation/Consultation steps regardless of the child having an existing IFSP, IEP or IPP and/or receiving services. Add Event Notes stating the date the referral was scanned to Disabilities. Disabilities Department will enter the “closed date” upon receiving the referral. Refer to 8.1.1.15 Mental Health (ASQ-SE) Follow Up
Description: Enter the type of screener (Ex: 36 month)
Results: Enter total score/cutoff score (Ex: 80/105)
Agency Worker: Ensure the Family Service Staff’s name is indicated.
Closed Date: If the Status was set to “Passed First Screening” enter the same date as the “Event Date”, if set to any other Status leave blank
Leave the rest of the boxes blank.
Click Save
For newly enrolled children:
After entering the ASQ SE-2 results into ChildPlus, the Family Worker must scan the completed ASQ SE-2 to the Disabilities scan folder.
