Needs & Service Plan
The Primary Caregiver must complete prior to the child’s entry into the classroom and every 3 months after that.
Family Worker will enter in ChildPlus within 3 days of completion.
Select The Participant from the Participants List In the Services Tab, select the Health Tab, and then select the Events section
Click on the Green “Add Event” button, then select the “Needs & Service Plan” Health Event
Event Date: Enter date parent signed the Needs & Service Plan
Expires: 3 months from the event date
Status: Select Completed
Agency Worker: Ensure the Family Service Staff’s name is indicated.
Leave all other boxes blank
Click Save
