1520 – Contact Info

These reports are the equivalent of the Child Care Facility Roster used for licensing purposes and must be retained for 3 years.

These reports are kept in the Licensing file and locked up with the child files at the center.

In the Reports tab use the search reports bar to search for 1520 and select 1520 – Contact Info

Enter the following information using the dropdown arrows:

Program Term:  Select HS/EHS of the program year you would like to view

Location: Shasta Head Start Child Development, Your Site, Your Classroom

Enrollment Status: Accepted and Enrolled

Flag/Group: Not Filtered

Program Option: All

Responsible Staff: All

Custom Filter: Not Filtered

Release To contacts

Do Not Release To Contacts

Emergency Contacts

Doctors

Dentists

 

Optional Data: Medicaid Eligibility, Insurance Information, Last DTaP, Allergy and Health Notes

Report Type: Detail

Group By: Site

Select The Individuals Button to access only the Individuals you wish to have data pulled for. If pulling the report for the entire class;

Click View Report

Always Preview your report before Printing.
