Attaching HV Records into ChildPlus
Filling out and Renaming Home Visit Records
Create a folder in your My Documents that you will Save As or copy the scanned Home Visit Records to (ex._ToCP).
Do one of the following:
If you filled out the home visit record on your laptop
1. Save the completed home visit record (ex. HV1_RCreek) in the folder you’re going to attach to ChildPlus from in your My Documents
2. See Attaching Files into CP directions below.
If you filled out a paper home visit record, scan Home Visit Record into your center _Scan folder.
1. Open your center _Scan Folder.
2. Find the home visit record you scanned.
3. Copy the scanned home visit record into the folder you created, you will attach the record into ChildPlus from this folder.
4. Rename each home visit file (ex. HV1_RCreek)
5. Delete files from _Scan folder.
Attaching Files into CP
1. Log into CP.
2. Select the child you are going to attach a home visit record to.
3. Click on Family Services tab.
4. Click on Attachments tab

5. Click on Add Attachment
6. Navigate to the folder you put the scanned home visit records in
7. Select the correct home visit record.
8. Click Open
9. Service Area: Ensure it reads Family Services
10. Attachment Type: Home Visit Record
11. Description: Type date of Home Visit (ex. 9/2/20)
12. Click Save
