CP Report 3011-Missing Health Event (Grid)
In ChildPlus Online, click on the Reports module. In the “Search Reports” box, type 3011 and select 3022 – Missing Health Event (Grid)
PARTICIPANT FILTERS
Program Term: HS/EHS (Current Year)
Program Option: ALL
Location: Uncheck “All” select Shasta Head Start Child Development and click on the arrow to display all sites, click on the arrow on your site and select the classroom(s) you wish to view. Select the centers/classes you wish to view
Enrollment Status: All
Flag/Group: Leave unchecked (Not Filtered)
Responsible Staff: All
Custom Filters: Not Filtered
OTHER FILTERS and OPTIONS
Health Event: Select Family Assessment or FPA (Family Goal) (you will need to pull this report for each health event, one at a time)
Show Participants: Who Are Completely Missing the Event
Begin Date: 7/1/ (Current Year)
End Date: Leave Blank

Report Columns to Show: All Selected
Show horizontal grid lines: Click Box
Show vertical grid lines: Click Box
Report Type: Detail
Group By: Classroom
Select “View Report”
Interpreting the 3011 Report Results:
If a child appears on the 3011 Missing Health Event (Grid) report after you select Family Assessment or FPA (Family Goals) as the Health Event:
- This means there is no Health Event record entered in ChildPlus for that child.
- In other words, the Family Assessment or FPA has not yet been documented for them.
- Action is required: the Family Worker or Home Visitor must complete the Family Assessment or FPA (Family Goals) with the family and enter the event into ChildPlus.
