Printing 4110 – Family Service History

1. Enter 4110 in the “Enter Report #” box and push enter.

2. Choose the Appropriate options for your caseload.

a. Program Term: EHS/HS & appropriate program
b. Agency: Shasta Head Start Child Development
c. Site: Choose your center
d. Classroom: Choose your classroom
e. Report Type: Detail
f. Order By: Participant Name

3. Check only:

  • Family Service Information
  • Family Partnership Notes
  • Closed Events
  • Event Notes
  • Family Service Actions
  • Action Notes

4. Initial Service Date Filter:
Begin: “Start of Program Year”
End: “Last day of current month”

5. Only include Family Service Events in which the Case Worker is:
All Checked

6. Only include Family Service Actions in which the Case Worker is:
All Checked

7. Event Types: Family Goal

8. Action Types: All Checked

9. Action Statuses: All Checked

10. Sort Actions by: Newest to Oldest

Reading the 4110 – Family Service History

1. Ensure each one of your families has a family goal entered.

2. Ensure that you update the Progress of the goal at the following times during the year.
