Dental Follow Up

After the failed dental exam has been entered by Health Department staff, Family Service staff will enter follow up actions in each time they talk to the parents regarding the failed dental exam.

Click on the failed Dental Exam to open it. Click on Add Action

Action Type: Follow Up

Action Date: Date you contacted parent

Description: Brief description of what happened (ie reminded parent)

Status: On-going SHS Contact Click Save

Family Service staff must follow up with the parent at least once a month and document it under the failed dental exam.

*When the family reports the child has gone in for treatment Family Service staff will obtain proof of the dental treatment from the dentist or family and scan to health for entry into CP. The treatment to the failed exam will be entered by the health team and a closed date will be entered if treatment is complete.
