Add Health Actions

1.  Click on the failed event

2. Click on “Add Action”

Scheduled Date: Enter an action with a scheduled date for 2 weeks or 30 days (depending on the screener, see specific event page, after the initial event date when a child needs a rescreen).

Action Type:  See each event for when to use Follow Up vs Treatment

Action Date:  Type in the date, the action was completed on

Description: Type a brief description (ie rescreen results)

Status Code: Select the appropriate status code (see event page for details)
