Family Worker and Home Visitor Monthly Summary Report
Monthly Summary Report
The Monthly Summary Report is completed by Home Visitors and Family Workers. The Monthly Summary Report is due by the fifth of the following month. At the top of the form, indicate the Center, Home Visitor/Family Worker name, and the Month/Year.
Use the Monthly Summary Report to summarize the following information for the previous month:
Reports to Review
Home Visitors and Family Workers must run the following reports in the timeframes indicated on the Monthly Summary Report. Home Visitors and Family Workers must review the information on the reports to ensure the integrity of the data in ChildPlus. Use the following reports to determine any missing, incomplete, or inaccurate data and make the necessary changes within ChildPlus.

- This ChildPlus report is used to ensure Employment, Job Training and Education PIR information for primary and secondary adult has been entered from the Family Demographics form.

- This ChildPlus Report is used to verify that there is initial and ongoing follow up for failed events. Instructions to run 3065
- Supervisors are responsible for monitoring the completion of the data entry described above (See SS/HT EOM Checklist).
REVIEW To Do Lists & Dashboards
Home Visitors and Family Workers should use this section as a reminder to check SHS Expired Events, SHS Follow Up Needed, SHS Immunizations Due, SHS Incomplete Requirements Weekly, and PIR Health Indicators monthly using ChildPlus To-Do Lists.
Attendance Concerns/Issues
Home Visitors and Family Workers will use this section to report out on any children/families that have ongoing attendance concerns. Be sure to indicate what the monthly attendance for the child/family is, whether any attendance notification has been sent out, and if there is a date scheduled for an Attendance Study Team (AST).
Orientation, Socializations and Parent Center Committee Meeting (PCCM) Attendance
Family Orientation and Parent Center Committee Meeting Attendance data will be tracked and shared with various stakeholders. FW/HV’s will complete this portion of the Monthly Summary with the total number of Parents that participated in each event. The Data Systems Coordinator will enter this data in ChildPlus.
Training or Support Needed
If a Home Visitor or Family Worker has a need for additional training or support, make sure to complete this section of the Monthly Summary Report. Give the topic for the support needed and a brief description of the need.
Submitting the Monthly Summary Report
Home Visitors and Family Workers should save a copy of their report and email it to the Family Services Coordinator, their direct supervisor, and their Area Manager. The Monthly Summary Report is due by the 5th of the following month.
