401(k) Enrollment
Each quarter, the HR Dept. will send out open enrollment notifications to all staff with information and a deadline to submit any new applications or any changes. Employees who are eligible for the first time will receive a full 401(k) packet from the Human Resources Department. To enroll in this plan, an application must be completed and submitted to HR (either the original or a scanned copy is acceptable) before the communicated deadline. A sample application with all required fields highlighted and helpful notations can be used to assist in completing the application.
All basic personal information is included in the application.

In the application process, the employee must decide which plan to participate in. There are two options: Roth and Pre-Tax. Both plans offer unique benefits.

Once a plan type is selected, a portfolio needs to be selected or created. Unified Trust provides multiples portfolios as well as the option to create your own portfolio. Only one portfolio can be selected.

OR

Once a portfolio has been selected, the frequency of account balancing must be chosen. The final piece of the application process is the designation of beneficiaries. In the state of California, if you are married, your spouse will be your primary beneficiary by default. If you do not want your spouse as your primary beneficiary, they must sign off to allow this change and the form must be notarized.
