401(k) Changes
Open enrollment for Shasta Head Start’s agency sponsored 401(k) plan occurs each quarter (January, April, July, & October). The Human Resources Department sends out mass communication to all employees with information including a deadline to submit any new applications or changes. Eligible employees can either begin contributing or make any changes to their current contributions during the open enrollment periods only.
Changes to contributions can be made utilizing the Participant Change Form and submitting it to the HR Department (either the original or a scanned copy is acceptable) before the communicated deadline. A Sample Participant Change Form has been created with all the required fields highlighted and helpful notations to assist in completing the form. There are two sections to this form – a personal information section and a contribution section.
The first section, “Your Personal Information”, should be completed if any changes to personal information has occurred.

The second section, “Your Savings Rate”, should be completed if any changes to the contributions need to be made. Only the top two options (Pre-Tax & Roth) apply to our plan.

Contributions may be stopped at any time but cannot begin again until a new quarter’s open enrollment period. To stop contributing, complete the Participant Change Form to zero out your contributions and submit it to the HR Department ASAP (either the original or a scanned copy is acceptable). Be sure to keep in mind the processing time for payroll. Any requests to stop contributions must be made at least 24 hours before payroll begins processing.
