Workers’ Compensation In-Person Treatment
In non-emergency cases where an injured employee is referred by the Nurse Triage Hotline to in-person care, they must complete a DWC-1 form before going in for treatment. The employee completes the employee section of the DWC-1 form and the supervisor completes the employer section. The employee will take the completed DWC-1 Form to Agile Occupational Medicine to be seen. The injured/ill employee will remain clocked in for their initial visit.
The supervisor should then contact and update the dedicated Human Resources representative and Area Manager with any new information. After being seen, the injured/ill employee must provide Human Resources with documentation that describes any information on clearance, restrictions, and/or any follow-up appointments. If the employee is not cleared to return to work, the HR Dept. will work with the supervisor and manager to determine what, if any, accommodations can be made. If the employee has restrictions, they may not return to work until the agency has determined if they can accommodate the restrictions.
Reminder: the time spent at the initial visit to Agile Occupational Medicine will be paid time. Any time spent in subsequent visits is unpaid time. Employees may use any accrued time off for unpaid time. It’s recommended employees schedule follow-up appointments (if necessary) around work hours.
