Employee Late/Missed Meal Break
Late or Missed Meal Break
The Employee Late Meal Break/Missed Meal Break form is used to document any instance where an employee was entitled to a meal break but either took one after the five-hour mark or did not take one at all. This form is used if the employee is not covered under the On-Duty Meal Period Waiver.
The supervisor must determine whether the employee was provided the opportunity to take at least a 30-minute meal break within the required timeframe.
If the employee was provided the opportunity to take their meal break but did not take it or took it late, fill out the “Missed/Late Meal Break – Employee Choice” section of the form.
If the employee was not provided the opportunity to take their meal break on time or at all, fill out the “Missed/Late Meal Break – Employer Required” section of the form.
The employee and the supervisor should both sign the form and scan or deliver it to their manager. Once signed by the manager it must be scanned to the HR Department.

Please follow the guidelines below when documenting late or missed meal breaks:
- For late lunches due to business needs, enter “RATIO” in the comments section of the OL (Out Lunch) punch.
- For late lunches not due to business needs, enter “FORM” in the comments section of the OL (Out Lunch) punch. Additionally, have the employee complete the Employee Late Meal Break/Missed Meal Break form and submit it to HR via scan by the end of the day.
- For missed lunches due to business needs, enter “RATIO” in the comments section of the OD (Out Day) punch.
- For missed lunches not due to business needs, enter “FORM” in the comments section of the OD (Out Day) punch. Additionally, the employee must complete the Employee Late Meal Break/Missed Meal Break form and submit it to HR by the end of the day.
