EED 3900- DRDP Summary of Findings
5.4.5 EED 3900
As part of the Shasta Head Start Ongoing Assessment process, classrooms that receive combined state funds, aka “state classrooms”, are required to complete the California state form EED 3900. The EED 3900 is a planning tool teaching staff will use to create and monitor class group goals.
Teaching staff will use the Learning Genie Class Planning Report, Group Profile Report, or both to identify two measures the whole class needs support in. These measures will typically be the two measures most of the children rate the lowest in.
Staff will follow the directions on EED 3900 to complete the fillable portion of the form, excluding the final section Follow-up and Reflection. The EED 3900 will be completed and scanned to the Education Folder within 2 weeks of the first DRDP rating period due date.
Throughout the year, as the children progress in their skills, staff may review the EED 3900 and add any notes or findings in the EED 3900 Follow-up and Reflections section. Following the final DRDP rating, staff will complete the Follow-up and Reflection section of the EED 3900, detailing what progress was made on the selected measures. Staff will scan the finalized EED 3900 to the Education Folder within 2 weeks of the final DRDP rating period due date.
Sample EHS and HS Preschool EED 3900 forms are linked below, as well as the fillable EED 3900.
