Personal Visit Planning Guide and Record

(Start HV 12 with New Children and HV 3 with Returning Children)

The Personal Visit Planning Guide and Record is used to intentionally plan each part of the home visit. It will help to individualize for each family and ensure the home visitor is prepared for the visit. The home visitor should use previous visit records, current goals, screening results, observations, and any additional information about the family to help develop visit plans.

Use of the Foundational Personal Visit Plans (HV 4-11) and the Personal Visit Planning Guide/Home Visit Record is an essential requirement for model implementation and curriculum fidelity.
