Alternate Location for Home Visit

The Alternate Location for Home Visit Plan is used when a family is not able or prefers not to have home visits in their home. This may happen for a variety of reasons, such as living with another family, staying in a motel, or other situations that make the home setting difficult for visits.

The Home Visitor will talk with the Family Services Manager to discuss the need for an alternate location and make sure the setting will still support a quality home visit, including parent-child interaction and family engagement.

If approved, the Home Visitor will complete the Alternative Home Visit Location Plan form, review it with the parent/guardian, and have them sign to show they agree with the plan. The Home Visitor will then sign the form and send it to the Family Services Manager for final approval.

Once it is signed, the form will be returned to the Home Visitor and placed in the child’s file.
