Home Visit Tracking Form

The Home Visit Tracking Form is used to document all home visits at a glance for each program year and used to complete the Home Visitor Monthly Summary Report (Part of the End of the Month (EOM) paperwork)

The home visitor will document the total annual home visit’s possible beginning with July (beginning of program year) or the first month enrolled through June (end of program year). If a child enrolls on week one of the program year, they will receive 46 annual home visits. If a child enrolls mid-year the annual home visit’s possible will be prorated to reflect enrollment date. Home visitors will also document how many home visits are possible for each month and how many of those possible were completed. Finally, home visitors will document whether the staff canceled the visit or if the family canceled the visit. Home Visitors must make up home visits they cancel and to the extent possible attempt to make up home visits canceled by the family.
