Service Providers at Center

When any child enrolled in Shasta Head Start qualifies for services from an outside agency, it is the responsibility of the Family Worker and/or Head Teacher Site Supervisor to review the Letter to Service Providers, the document Protections for Privacy of Child Records and Program Data and the Acknowledgment of Requirement to Report Child Abuse | Spanish. After reviewing the Protections for Privacy document and the Acknowledgement of Requirement to Report Child Abuse, the Family Worker or Head Teacher/Site Supervisor must obtain a signature from the service provider and store a copy of the forms in the Service Provider sign in binder at the center. The signed documents can be shredded after storing for one program year.

The purpose of the Monthly Confirmation of Treatment form is to ensure that every child is receiving the services that are offered in their Individual Family Service Plan (IFSP) or Individualized Education Program (IEP). It is the Family Worker and/or Head Teacher Site Supervisor responsibility to ensure that the form is completed by the Service Provider each time services are conducted at the center. At the end of each month the form must be scanned to the Disabilities and Mental Health Department who will enter the data into ChildPlus. The family must be provided with a copy of the form, and the original will be kept in the Disabilities section of the Child File. The Service Provider Binder must be kept in a locked file cabinet at the end of the day to protect Personal Identifiable Information.

The Education Staff will review the notes left by the Service Provider, and implement individual strategies in the lesson plan, with the intent of supporting the child to reach their goals.
