Behavior Incidents
Behavior Incidents occur when children exhibit severe behavior(s) that require the submission of a Behavior Observation Report (BOR) to the Area Manager and Disabilities/Mental Health Department.
Behavior Incidents could include:
- Aggression to another child or adult that results in an Ouch Report or Supervisor’s Report of Accident (including kicking, hitting, biting, scratching)
- Running out of classroom, off the playground, or away from group or supervised area without responding to the calls of the adult
- Intentionally injuring self in a manner that may cause serious harm (e.g., severe head banging, biting self, etc.)
- Any behavior that results in staff following their Behavior Crisis Intervention Plan or an Unusual Incident Report
- When a child continues to engage in problematic behavior despite efforts to redirect and/or to use alternative skills as prescribed on the Positive Behavior Support Plan of Action
It is not considered a Behavior Incident if the behavior is developmentally expected. These behaviors may be tantrums, inappropriate language, hitting, property destruction, or disruptive behavior. An example of developmentally expected behavior is two two-year old children tussling over a toy.
The Head Teacher or Site Supervisor must thoroughly review and sign the BOR if it qualifies as a Behavior Incident prior to reviewing with the parent/guardian. When writing the Behavior Incident Report (BIR), be sure to include the reason the BOR is considered a Behavior Incident in the comments section. The Head Teacher or Site Supervisor must thoroughly review and sign the BIR prior to sending it to their Area Manager for approval. Upon Area Manager approval, the Teacher/Primary Caregiver or Family Worker will review the BIR with the parent/guardian and initial and date the BIR once reviewed. The BIR will be scanned to the Disabilities/Mental Health Dept. within 48 hours of incident. Staff will work with the parent/guardian to obtain their signature acknowledging the incident. A copy of the report will be offered to the parent/guardian and the signed original will be kept in the child file.
