Child Files
Teachers need to review their child files prior to the start of the school year. Child files are created at enrollment and stored in a locked filing cabinet in the office and are to remain there.
For information on what is included in the child file, refer to the SOP Preparing Child Files.
Teachers should check each child file to ensure that the Education Cover Sheet has been included in it.
The coversheet lists all of the things that need to be done either prior to the start of school and after school starts. Everything listed on the cover sheet must be completed and included in the child file at the end of the school year when the child file is closed.
The Education Coversheet is generated and printed from ChildPlus as part of the Enrollment Packet.
Each Education Cover Sheet indicates the:
- Documents which must be reviewed with the family
- Date completed with initials of completed by individual
- Date entered into Child Plus (if applicable)
- Follow-up/Referred date (add action in Child Plus)
- Comments
Teachers should review each section of the child file and document information on the Child File Summary form. The Child File Summary is then kept in the Teacher Binder. This allows the Teacher to access pertinent information without having to leave the classroom.

- Health (Pink) (HDNA/Dental/Hearing/Vision)
- Disabilities (Orange) (referral/CST/IEP/IFSP)
- Family Services (Green) (Family goals)
