Travel File

The travel file holds the weekly home visit paperwork with families enrolled in the home-based program.

Home Visitors will receive a folder for each family at the beginning of the program year so they can create a travel file prior to the first home visit. The travel file should include:

  • A copy of the Emergency Card (in admission paperwork printed from ChildPlus)
  • Individual Home Base Child Goals (once created) (link)
  • Family Services Section (Green section) (Print from CP)
  • Personal Visit Planning Guide/Home Visit Record
    • The previous week’s record and the current record will be found in the travel file for planning of weekly home visit
    • Past/completed weekly home visit records will be filed in the child file (stored at the center) in the Child Development section.
  • Screeners/tasks to be completed
  • Resources/Family Handouts

At the end of the program year or when a child drops mid-year, the contents of the travel file will be combined with the contents of the child file and “closed”.
