2.5-Year-Old

Transition from Early Head Start to Head Start

To ensure the most appropriate placement and service following participation in Early Head Start, such programs must, at least six months prior to each child’s third birthday, implement transition planning for each child and family that:

  1. Takes into account the child’s developmental level and health and disability status, progress made by the child and family while in Early Head Start, current and changing family circumstances and, the availability of Head Start, other public pre-kindergarten, and other early education and child development services in the community that will meet the needs of the child and family; and,
  2. Transitions the child into Head Start or another program as soon as possible after the child’s third birthday but permits the child to remain in Early Head Start for a limited number of additional months following the child’s third birthday if necessary for an appropriate transition.

Important Notes:

  • For more information, refer to the EHS 2.5 (30 months) Year Old Transition section (pages 2-3) of the Comprehensive Procedures for Transitioning Children and Families form. The form includes a list of the tasks and activities which must occur along with the timeframe, staff responsible and applicable forms to be completed for each task/activity.
  • The Notification of Family Transition form is not completed for this type of transition (EHS to HS).

Additional Information

  • If child has a disability/IFSP, they’ll begin transitioning to an IEP (for preschool/elementary school) at the age of 2.5-years-old.
  • Use the EHS Required Forms Tracking Instrument  form to track the 2.5-year-old/30-month due date/completion date. For more information, refer to 5.3.28 Off-Floor Planning Time 
  • The Family worker or Home Visitor must track when items related to 2.5-year-old activities are due in Child Plus. The system auto populates the completion date based on the child’s birthday.
