Other In-House Transitions

When a child transfers from one Shasta Head Start program, location, or classroom to another Shasta Head Start program, location, or classroom:

  • The sending Family Services staff and Child Development staff meet to begin the paperwork process. Information from the Child File is used to complete the Individual Transition Plan (ITP) | Spanish, including:
    • Family profile and goals.
    • Child strengths and development.
    • Health status.
    • Ideas for easing the transition.
    • Possible activities for the action plan.
  • The sending Family Service staff schedules a transition meeting with the parent(s) and the sending and receiving Family Service staff and Child Development staff. Prior to the meeting, consider whether the parent(s) require interpreter services.The Family Service staff and Child Development staff should elicit additional information from the parent(s) and partner with the parent(s) to develop the action plan for the transition during the ITP meeting. For more information about ITP meetings, refer to the ITP Meetings topic.  Enter ITP information in the Family Services module of Child Plus after the ITP is developed.The receiving Family Worker, Teacher, or Home Visitor should know about the following items before the child transitions:
    • English Language Learner.
    • Individualized Education Plans or Individualized Family Service Plans.
    • Behavior Plans.
    • Health and Nutrition Plans.
    • Cook’s Diet Orders.
    • Attendance Contracts.
  • The sending and receiving Family Service staff and Child Development staff follow the action plan as developed with the parent and detailed in the ITP. Resources needed to ensure a smooth and comfortable transition should be provided.
  • Prior to the transition, the sending Family Service staff:
  • If it is an end of the year transition, the Family Service staff completes the ITP and inputs 1st and 2nd choices in Child Plus Enrollment module no later than last business day of April. 8.4.08 CP Update a Family’s Accepted Enrollment Record
