Develop ITP
The ITP form contains 5 critical sections that provide supporting information for the development of an appropriate transition plan for the child. These sections include:
- Family Profile and Goals: This section provides background on the family, including any strengths, weaknesses, or opportunities that affect the success of the child. The family’s goals are considered and any specific areas of emphasis are noted.
- Summary of Child’s Strengths and Development: This section summarizes the child’s strengths in the classroom and overall developmental status and progress. While it is critical that this section offer an honest evaluation, the emphasis should be positive aspects of development and status, rather than a list of negatives or challenges.
- Summary of Child’s Social, Emotional, and Heath Status: This section summaries the child’s social, emotional, and mental and physical health with emphasis on strengths and opportunities that might impact his or her transition.
- Ideas for Easing Transition: This section provides child-specific suggestions for easing the transition and helping the child move from a familiar environment to an unfamiliar environment. As noted in the section, this might include implementing familiar routines/rituals, objects, and favorite activities/songs in the new environment to provide familiarity.
- Action Plan/Timeline for Transition Activities: This section defines the specific steps of the ITP, including the concrete actions and responsibilities of the parents and the Shasta Head Start staff, along with specific deadlines. These steps should provide clear, unambiguous guidance regarding the tasks assigned to each party, supported by further discussion and clarification during the ITP Meeting, if required.
