Receiving Referrals

Many of our family referrals come through the following partner agencies, although the list is not limited to these:  

These referrals are usually faxed to us using the Community Referral Form.

When SHS receives a community referral, an Enrollment Coordinator enters information from the referral into the Recruitment and Referral module in ChildPlus and attaches the referral form to the family entry within the module.

The Enrollment Coordinator calls the family to explain the program, discusses documents they may need to gather, and schedules an eligibility interview.

If the referral is a “High-Risk Referral” and the family meets all the eligibility requirements, they are assigned priority points when their child is placed on the waitlist. See Head Start and Early Head Start Eligibility Requirements for the breakdown of eligibility points.

Disability Referrals 

We receive most of our disability referrals from Far Northern Regional Center, a Part-C agency. Occasionally, we receive referrals by word of mouth or phone call from the school districts.  

Although the family still needs to meet all the eligibility requirements, with proof of an IEP or IFSP, they are assigned priority points during the eligibility process.  

When SHS receives a referral from Far Northern Regional Center, the Disabilities Coordinator employs ChildPlus to determine the child’s status in our program.

If the child is enrolled in the program and the referral includes an IFSP, the Disabilities contacts the Family Worker and Site Supervisor at the center to notify them and provide the information.

When the child is on the waitlist or the family is in the eligibility process, the Disabilities Coordinator contacts the ERSEA department to ensure the referral and IFSP are added to the family’s information so priority points may be assigned.

If SHS has no data on the family, the Disabilities Coordinator contacts the ERSEA department to notify them of the referral. The Enrollment Coordinator enters the information from the referral in the Recruitment and Referral module and contacts the family to schedule an eligibility interview. Once the family completes the eligibility process and is added to SHS’s waitlist, priority points may be assigned. Note:

The Disabilities and Mental Health Manager must approve priority points for any IFSP/IEPs received by SHS.
