Returning Family Checklist

  1. Open a family in CP
  2. Click on the “Enrollment” module
  3. Click on the “Returning Family Checklist” tab
  4. At the top right of the module, click “Add Returning Family Checklist”
    1. One record per program year – Be sure to select Next Year program term.
  5. Scan to Receiving FW/HV & _Returning_Family folder.
  6. File any signed Declined Slot RFC’s under Blue Enrollment section after any Notice of Family Transition.
  7. Module will be changed to View only each year on a date to be determined by the ERSEA Manager.

A: BASIC INFORMATION

  1. CP will auto-fill the date
  2. Select what PY this checklist is being completed for
  3. Sending FW/HV will be the name of the person completing a majority of the checklist
  4. Specify which site & class the child is coming from

CHILD INFORMATION

  1. Enter child’s First & last name & DOB
  2. Enter Primary Parent’s Name
  3. Individual Child Needs
    1. Check all that apply

B: PROGRAM OPTION’s

  1. Enter Data Entry Date
  2. Check box for either Early Head Start or Head Start
  3. Check the FCC or State box if child is wanting to attend a center that is part of one of those collaborations
  4. Select either a part day(AM or PM), full day, or home based option
  5. Use drop down choices to select which site & classroom is the family’s 1st choice.
  6. Repeat steps 8-12 for family’s second choice.
  7. Scan Form to:
      1. Receiving FW/HV
      2. _Returning_Family folder
  8. File any Declined Slot in Blue Enrollment section behind Notice of Family Transition.

D: SELECTION

  1. Receiving FW/HV will add family to their Selection caseload for review with AM & Site Supervisor.
  2. FW/HV enters date on which they were selected or denied for their program option.
  3. If selected, FW/HV will also enter date & classroom assignment.
  4. PLEASE RETURN TO AREA MANAGER IF 2nd CHOICE IS NOT SELECTED!!!
