Individual Transition Plan (ITP) Meetings
An Individual Transition Plan is created when an enrolled child is transitioning from one Head Start program to another Head Start program or out of the Head Start program (e.g. Kindergarten). An ITP is created whether the child transitions during the school year or at the end of the school year.

Center-based
For center-based children who will be transitioning at the end of the year (e.g. to kindergarten), the ITP meeting is discussed at either the Second Family Conference or during the Final Home Visit.
- If a center-based child is transitioning during the school year, for example from one center to another center, an additional Family Visit would be scheduled to discuss the ITP plan for the child.
Home-based
For home-based children who will be transitioning at the end of the year (e.g. EHS to HS), the ITP meeting would take place during a weekly home visit towards the second half of the program year (Jan/Feb).
- If a home-based child is transitioning during the school year, for example from a home-based program to a center-based program, an ITP meeting would take place as part of a regularly scheduled weekly home visit.
- The ITP meeting may take place outside of the home, based on the actual transition scenario (e.g. the child is transitioning to a center-based program).
The ITP Meeting
When transitioning during the school year, the ITP Meeting between the sending staff, receiving staff, and the parent(s) is usually the same meeting where the ITP is developed. This single meeting achieves the objectives of both developing and reviewing the ITP to ensure that all parties are in agreement with the goals and steps of the transition.
When transitioning at the end of the school year, the ITP meeting is a separate event between the sending staff, receiving staff, and parent(s) and should not be combined with the second conference.
